Objectives: Describe the clinical characteristics of severe pediatric patients

Strength of evidence: Low (8 patients, monocentric, descriptive)

Methods/publication type:

  • A monocentric observational cohort study

  • 8 patients 2 months to 15 years old, 6 boys

  • Severe COVID-19.

  • Severe case : FR> = 30 or SpO2 <93% in AA or PaO2 / FiO2 <300 mmHg (5 cases / 8)

  • Critical case: Mechanical ventilation or septic shock + failure of organs (3 cases / 8)

  • Wuhan

Highlights :

  • Polypnea (8/8), fever (6/8) and cough (6/8), groans on auscultation of the lower lobes (5/8) crackling (3/8)

  • Bilateral pneumonia (6/8) / multiple patch-like shadows (7/8), ground-glass opacity (6/8)

  • Decreased CD16+CD56 (4/8) and Th/Ts*(1/8), increased CD3 (2/8), CD4 (4/8) and CD8 (1/8), IL-6 (2/8), IL-10 (5/8) and IFN-γ (2/8).

  • Increased C-reactive protein, procalcitonin and lactate dehydrogenase (6/8)

  • Treatment: High flow oxygen therapy (6/8), Mechanical Ventilation (2/8) / have all had antivirals (virazole, oseltamivir, interferon)

  • Evolution: 3 patients still in intensive care on February 24, 5 others discharged from hospitalization

  • Most common symptoms: polypnea, cough, and fever

  • Pulmonary images of multiple patch-like shadows and ground-glass opacity
